Warranty Claim Form

To make a warranty claim:

  1. Complete the Service Request Form below.
  2. The claim will then be evaluated.
  • If the part is not covered under warranty, you will be notified.
  • If the part is covered under warranty, the order will be processed and a confirmation email will be sent.
  • If the part requires installation, it will be sent to the service technician in the area and you will be contacted by an ergoCentric Representative.
  • Product information

    Every ergoCentric seating and furniture product is assigned a serial number, which is stored in our database along with all the specifications and relevant information for that product. You can find the serial number under the seat pan of your chair, on the mechanism. For electric height-adjustable desks, the serial number is located underneath the table, on the frame.
